Pathways Support Worker

Full-time, fixed-term for 6 months + Pukekohe + Monday - Friday

Work alongside tāngata whaiora to maintain physical, emotional & spiritual wellness. Assist them with meal prep, clean-up & other daily living tasks.

About the role:

This is an intensive team that supports whaiora with complex physical and mental health problems. You will work alongside the clinical team and with other health services. You will support the whaiora with their shopping, daily activities, GP appointments etc. A mental health background is preferable.

This is a fixed-term position for 6 months. You will be working full-time, 8am – 4:30pm, Monday – Friday. You will start and end your shift at our residential service in Pukekohe, Auckland.

Responsibilities include:

  • Advocate and support whaiora to be well and thrive.
  • Connect them with resources that support them in living well and assist them in building resilience
  • Help connect with their communities, building the relationships that matter most
  • Foster positive change for a more positive life
  • Provide medication support for our whaiora
  • Support Whaiora with their appointments

You:

  • Understanding and commitment to principles of recovery
  • Current full NZ driver's licence
  • Current first aid certificate
  • Applicants must hold the New Zealand Certificate in Health and Wellbeing (NZCHW) or a recognised equivalent
  • Support Worker experience or similar
  • Sound knowledge and understanding of the primary care sector
  • Knowledge of, and demonstrable commitment to, the principles of the Te Tiriti o Waitangi
