Youth Comprehensive Community Services (CCS) is a voluntary program designed to provide in-home mental health support and services to youth and their families who have mental health and/or substance abuse diagnosis. CCS helps individuals live their best lives by providing client-centered and selected services tailored to their unique needs including case management, therapy, mentoring, and other services as needed to meet self-identified goals.

The Youth CCS Service Facilitator is responsible for community-based support for the mental health and behavioral needs of youth and families. Youth CCS Service Facilitators support families through connection to community resources, client-centered services, and a strengths-based approach to healing.

Responsibilities:

  • Complete clinical assessments, including functional screens for all clients at intake and on a yearly basis.
  • Develop and implement client treatment plans, interventions, skill training, and crisis management.
  • Encourage family investment and participation in treatment planning and ongoing services.
  • Seek client input and voice in service planning and provision and follow up with service providers to ensure quality care and goal progression.
  • Develop a network of community support and resources and connect clients with these resources as appropriate.
  • Collaborate with multidisciplinary teams to ensure a team approach to services and effectively communicate with service providers.
  • Engage in discharge planning for each client throughout the service process.
  • Establish and maintain a working knowledge of the WCHSD programs, resources, and supports.
  • Complete all necessary paperwork in a timely manner and follow all standards within DHS 36.

Qualifications and Competencies:

  • Master\'s or Bachelor\'s Degree in Social Work (APSW preferred) or a related field required.
  • At least one (1) year of experience working in the Mental Health &/or Substance Abuse field, preferred.
  • Ability to work in-home with participants, as well as coordinate services with outside providers within the community.
  • Ability to multi-task and prioritize work within the community, in conjunction with the timely completion of documentation.
  • Ability to take initiative and act quickly and independently when the situation demands it; sees a need and takes appropriate action without being prompted or reminded; demonstrates an ability to effectively deal with crisis situations.
  • Understanding of and experience providing services through person-centered and Trauma-Informed Care lenses; experience with DBT, preferred.
  • Ability to exhibit professionalism while working effectively and harmoniously with staff, program participants, social service agencies, legal parties, and the public.
  • Demonstrate an understanding and practice of cultural humility and the ability to work well with individuals from diverse economic, racial, and ethnic backgrounds and LGBTQ+ identities.
  • Pass a pre-employment background check.
  • Valid driver\'s license and automobile with insurance.
  • Proficient in Microsoft Office (including Outlook, Word, and Excel), web-enabled databases, and computer file systems.
