Responsibilities:
Ability to support individuals to live independently in their community
Work as a mentor and build relationships with individuals to assist them to connect to their community
Ability to support individuals with any activities of daily living that are needed to live independently. This can include, but is not limited to cleaning, cooking, laundry, medication support, bill paying and personal care support
Ability to develop community relationships that fosters connections between the person supported and community organizations (volunteer or join a club for example)
Ability to make decisions and work independently
Other Experience and Qualifications:
Qualifications:
High School graduate or equivalent
At least two (2) years of experience working with persons with disabilities or in a mentor/teaching position that assists individuals in learning skills and providing them independently
Experience and understanding of community resources and methods of teaching independent living skills
Valid driver\'s license with good driving record and insured vehicle available for daily use
