As a Social Service Designee, you will be responsible for:

• Pre-admission screenings, assessments, discharge planning
• Schedule and mediate multidisciplinary care plan meetings
• Collaborate with the team to meet the individual psychosocial needs of our residents

Ideal candidates must possess:

• Possess strong computer skills and social skills
• Possess strong time management and organizational skills
• Desire for a short-term temporary role
