Service And Transition Navigator

The Toowoomba Integrated Mental Health Hub is an innovative new model of integrated stepped care, transforming the way mental health and wellbeing support is delivered across Queensland. Wellways, together with our consortia partners Each, Lumsden Psychology, QPASTT and Goondir will provide seamless, free and voluntary services built on the principles of no wrong door, community inclusion and consumer choice and control.

About the role

As a Service and Transition Navigator, you will use your personal lived experience of recovery to walk alongside people as they navigate change, connect with supports and move towards what matters most to them. This role is grounded in hope, mutual learning and strong relationships, using positive self-disclosure and role modelling to support recovery and wellbeing.

Working closely with a multidisciplinary team, you will provide practical, strengths based peer support that helps people overcome day-to-day barriers, build confidence and stay connected to the services and communities that support them.

In this role, you will support participants, families and carers to navigate local services and transitions in a way that is person-centred, inclusive and responsive.

You will also:

  • Deliver intentional, recovery-oriented peer support and service navigation using the Intentional Peer Support framework
  • Support intake, planning and coordinated transitions
  • Facilitate peer groups, build capacity and support engagement with community
  • Collaborate with clinical and community partners to support planning, safety and coordinated care

Some pre-employment requirements

  • Certificate IV/Diploma in Mental Health or Peer Work, AOD or tertiary qualification in social science or 1+ year relevant work
  • Experience in mental health sector or intentional Peer Support Training
  • National Police Check
  • Current Working with Children Check - employment
  • NDIS Workers Screening check
  • Evidence of right to work in Australia
  • 100 points of identification
  • NDIS Workers Orientation module certificate – free online course

Toowoomba Integrated Mental Health Hub will provide the community with a free, safe and welcoming space to access mental health advice, supports and services.
