We are seeking a dynamic and community focused individual to join our team as an Outreach Carer Services Navigator. This exciting new role is designed to raise awareness of Carer Gateway services and enhance support for carers across the Lithgow and Nepean Blue Mountains region through a mobile outreach hub. Working at the intersection of community development, engagement, and direct carer support, this role will build strong relationships with local organisations, schools, health services, and community groups to embed carer support into existing local services. As a key community presence, you'll lead outreach campaigns, participate in local events, and provide on the ground advice, intake, and planning support for carers.
In this role, you'll play a vital part in identifying hidden carers, facilitating peer support groups, and creating sustainable referral pathways in collaboration with local partners. You'll also deliver structured, yet flexible peer support programs based on the CHIME framework, empowering carers to connect, build resilience, and access essential services. If you're passionate about community connection, enjoy working independently, and thrive on meaningful engagement, we'd love to hear from you. A current driver's licence and willingness to travel regularly across the region are essential.
