Fauquier Health is proud to offer a wide range of screening and diagnostic imaging services to provide customized care for each individual patient's needs. As a Patient Navigator, you will work in collaboration with the multidisciplinary care team to ensure effective communication, support quality outcomes, and empower patients to become active participants in their own care. This position will serve as a liaison throughout the facility and within the community offering clinical expertise, education, consultation, and advocacy on behalf of this patient population. Initial efforts will focus on supporting patients with colon, urologic, and breast cancer diagnoses with plans to broaden navigation support as service needs evolve.

How you'll contribute A Patient Navigator who excels in this role: Provides education and resources to patient and families about their cancer type and treatment at initial consult and throughout the continuum of care. Supports patients in developing personalized, patient-centered care plans in collaboration with their healthcare team. Assists with scheduling tests, treatments, and specialist appointments; coordinates follow-up care and transportation as needed. Refers patients to appropriate support services. Conducts outreach and educational activities related to cancer awareness, prevention, and screening. Services as patient advocate for the physical and emotional well-being of patients and their families.

What we're looking for Applicants should have previous experience in health care, community health, nursing, or related human service science. High School diploma or equivalent required. Basic level of background clinical knowledge and medical terminology related to oncology treatments required.
