Responsibilities:
Provides quality care, training, support, and supervision to individuals in a center-based or community-based program. Monitors and maintains the safety and wellness of individuals. Provides a wide array of activities and experiences that will enhance the lives of individuals. Provides recreation and socialization to individuals in a variety of community settings. Provides transportation for community activities. Maintains individual records and documentation according to all applicable standards. Hours of the program may include weekdays, weekends, and evenings.

Minimum Requirements:
M1: High school diploma or equivalent
M2: Valid driver’s license and safe driving record
Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.
Annual Salary Range: $32,493 - $56,863
3% Increase for Certified Nursing Assistant
