Provides guidance to clients with a knowledge of social and vocational case management and counseling methods to develop individualized service plans and perform psycho social assessments to determine personal, social, health and economic needs. Assists clients by independently identifying and referring them to available federal, state, local, and private resources to assist with their needs. Educates clients on clinical and other relevant procedures and coordinates client care and community resources.
Minimum Qualifications
Experience and knowledge of: family services principles, practices and ethics; development of treatment plans to deliver services and locating and securing resources; knowledge of infant and child development; parent-child dynamics; child abuse, neglect, and trauma impact • Experience and knowledge of: individual counseling and relationship building; effective communication; group leadership; documentation, data entry, and record keeping • Ability to promote appropriate programs; be a positive role model and mentor for clients, providing emotional support, health and safety information, and community referrals • Strong verbal and communication skills; skill in interviewing to gather, interpret, and use data appropriately; case management and excellent organization skills; ability to handle multiple tasks and work effectively as a team member • Ability to adhere to confidentiality and handle sensitive information • Computer skills to include web-based case management documentation
