HIM Deficiency Analyst

The HIM Deficiency Analyst is responsible for the concurrent and retrospective review of medical records for timely required documentation to ensure compliance with accrediting bodies, accreditation agencies, regulatory bodies and medical staff rules and regulations. Provides assistance to providers to resolve deficiencies and uses data management techniques to trend areas of improvement opportunity. The HIM Deficiency Analyst works with HIM and other department staff to ensure the accurate and timely completion of patient medical records. Provides clerical support and consult for other HIM functions. Completes weekly physician suspension of privileges report and manages deficiencies both currently and post-discharge to ensure that all required documentation is present, and that all entries have been properly authenticated before the medical record is determined as complete. Major function requires analysis and interpretation of data using inferential methods to affect departments desired results producing quality of work in a timely manner to assure accurate and efficient chart completion for accurate patient care. By using critical thinking and deductive reasoning skills, this position will identify the root cause of a problem in all functions of deficiency analysis. This position requires a high level of detail and must maintain strong independent work habits with the ability to effectively communicate with all levels of associates and clinical staff regarding Health Information Management processes.

Responsibilities And Duties:

  1. Initiates and performs technical procedures to analyze, or reanalyze, and monitor for medical records deficiencies according to Joint Commission and Medical Staff Rules and Regulations for regulatory requirements.
  2. Assists in the execution of physician notification processes regarding medical record documentation deficiency, delinquency, and suspension, including deficiencies impacting the DNFB.
  3. Execute the task of closing and verifying the episode of care by ensuring that all pertinent clinical documentations are in the medical record.
  4. Perform the re-analysis and loose scanned document analysis separately from discharged medical records.
  5. Identifies providers with outstanding deficiencies, updates the providers' statuses, and update the episode of care for the chart completion.
  6. Identify charts and analyze charts for concurrent analysis and assign deficiencies as required.
  7. Tracks and meets productivity, timeliness, and quality standards and goals set by the department.
  8. Communicates cooperatively and constructively with physicians, clinicians, and physician office staff.
  9. Maintains compliance with pertinent OhioHealth policies and procedures.
  10. Manage the suspension process to ensure that all documents are complete and accurate in the episode of care to complete the chart.
  11. Communicate with the Medical Staff Team to escalate the list of providers who are pending suspension.
  12. Assist and collaborate with providers with issues preventing them from completing the deficiencies and missing documents.
  13. Escalate issues to the management team to obtain assistance with providers with deficiency and missing documentation concerns.
  14. Gathers statistical data regarding individual physicians' record documentation and completion performance for reporting to the Medical Staff Office, physician leadership and any other hospital committee as needed.
  15. Assists medical staff members in all aspects of record completion.
  16. Understands and performs corporate suspension procedures for the individual facilities.
  17. Processes physicians' records that have been placed on leave of absence by Medical Staff Services.
  18. Identifies document(s) needing to be re-scanned when image quality is poor and routes the account to the appropriate work queue for processing.
  19. Assists in cross-training other employees as necessary.

Minimum Qualifications:

  • Associate's Degree, High School or GED

Additional Job Description:

SPECIALIZED KNOWLEDGE Demonstrates knowledge of medical terminology as it relates to medical records is required. Excellent communication skills, written and verbal. Demonstrates critical thinking skills, proficient and/or experience with an electronic health record and/or a document imaging system, problem-solving skills, and excellent multi-tasking skills. Must have strong computer skills, including proficiency in Microsoft Word, Excel, and Teams. Exceptional organizational skills with attention to detail, ability to problem solve, organize, and prioritize workload and to think and work independently. Ability to utilize deductive reasoning by researching, identifying, and retrieving patient clinical information from a variety of electronic sources and adapt to changing technology. Must be able to understand and interpret Joint Commission and other regulatory standards. Ability to be a team player in a team-oriented environment.
