Position Summary
Telework position, can be located anywhere in the US. This position is responsible for member-facing and provider-facing outreach to support Medicare Part D stars initiatives including any current Part D measures. Calls are made to members on the West coast so work hours will be during appropriate call times for the Pacific Time (PT) zone.
Responsibilities of this position include:
Execute clinical pharmacy programs
Coordinate strategies for increasing adherence
Utilize motivational interviewing and health coaching to impact member behavior
Outreach to members 90% of the day
Ensure effective collaboration with internal and external matrix partners to support clinical programs
Work collaboratively with leadership and management to strengthen business and operational processes
Other duties as required
Required proficiency in Microsoft Excel, Teams and Sharepoint.
This position could involve travel up to 10%
Required Qualifications
1+ years of experience with Medicare Stars, managed care or population health
2+ years of customer or patient-facing experience
Enhanced knowledge of pharmacy and claims processing
Excellent organizational and analytical skills
Superior communication skills with patients and clinicians
Ability to prioritize daily to monthly deliverables
Computer Skills: Proficient in Microsoft Excel, Microsoft Teams, and Sharepoint
Preferred Qualifications
Certified Pharmacy Technician (CPhT), Technician Trainee license, Licensed Practical Nurse (LPN), Certified Medical Assistant (CMA), or equivalent clinical training
Knowledge of formulary designs, prescription benefit trends
Bilingual in English and Spanish
Education
High School Diploma, Associates degree preferred

Pay Range
The typical pay range for this role is:
$46,988.00 - $112,200.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
