Essential duties and responsibilities include the following. Other duties may be assigned. Initial contact for all referrals. Serves as a coordinator of services including, but not limited to: identification, assessment, evaluation, intervention planning, implementation of intervention, and transition planning. Makes sure all procedural safeguards are met on behalf of the parent/child. Complete initial evaluation to determine eligibility in the program and complete ongoing assessment of both typical and atypical development, to ensure continued program eligibility. Interpret evaluation results with the family and provide assistance in understanding the child’s development levels and progress. Identify and educate on developmental needs, strategize methods to bring positive outcome for the child. Write developmental assessment reports. Provides parent primary coaching on overall development. Identify child’s area of need/concern and researches ideas and strategies, shares ideas with family and team at the special instruction visits. Assist families and caregivers through education and other training techniques in developing speech, motor, and cognition, leisure-time activities, socialization, behavioral management, and activities of daily living that can be incorporated through daily routines. Facilitate, develop, and write Individualized Family Service Plan (IFSP) with family and therapist input, with consideration for the overall developmental, social, emotional, and physical needs of all children in the delivery of Birth to 3 services and plan development. As part of a team, with family input, decide who the primary provider/coach for a child will be, for those who could benefit from more than one direct service (e.g., occupational therapy, physical therapy, speech therapy, and special instruction). Discuss new strategies and supports to help family to be incorporated in upcoming visits, and upcoming transitions. Collaborate and recommend a plan on ways to overcome any barriers. Update team on any recent medical changes or recommendations for additional evaluations. Examine ways to support child/families social and emotional needs. Educate and support family on all transition processes from Birth to 3 including program transition into school program. Complete all needed transition documentation. Coordination and attendance at area schools for transition meetings. Act as an advocate and a support for parents through this process. Discuss progress towards meeting goals at weekly team meetings. Make therapeutic recommendations for equipment and complete letters of necessity when required. Communicate with doctors regarding evaluations and recommendations for ongoing services. Maintain liaison relationship with local education agencies, physicians, public health department, and other community resources. Work a flexible schedule to meet the needs of the families and meet families in their natural environment (home and community settings). Provide community education/consultation/intervention regarding Department services. May develop ideas and create educational posts for social media. Maintain case records containing pertinent, accurate and current information; prepares correspondence, reports and other records as required.
This job has no supervisory responsibilities, but contributes to the oversight and guidance to professional and para-professional staff of the Health & Human Services Department, in accordance with the organization’s policies, procedures and applicable laws.
