Support Service Workers Needed

The Support Service Workers will provide services and other activities as designated in the Individual Support Plan (ISP). This will be done in a manner consistent with efficiency, cost effectiveness and regulatory compliance.

Responsibilities

Compliance:

  • Follow Values of Everyday Lives' Policy.
  • Submit HR documentation on time.
  • Wear Identification badge.
  • Comply with Agency requirements.

Support and Relations:

  • Enhance individual's growth and development.
  • Follow Individual Support Plan (ISP) and Agency policies.
  • Prioritize customer service.
  • Communicate courteously with Managing Employer.
  • Report and document complaints.
  • Maintain customer relationships.
  • Strive for quality and satisfaction.

Safety and Documentation:

  • Report safety issues and injuries.
  • Follow infection control and safety standards.
  • Complete infection control surveys.
  • Document care and observations.
  • Notify changes in individual's condition.
  • Complete and submit records on time.

Participation and Confidentiality:

  • Join Agency committees as needed.
  • Support Agency mission and philosophy.
  • Attend required training.
  • Protect confidentiality of individuals, Managing Employer, and Agency.

Must be able to read and write at an 8th grade level in order to complete service records.

Agency with Choice Services: Companion Services W1727: Level 3 Companion services are provided to individuals living in private residences for the limited purposes of providing supervision and minimal assistance that is focused solely on the health and safety of the adult individual with intellectual disabilities.

Must successfully complete background checks to include federal and state mandated clearances, employee health screening, and post job offer drug testing.

  • Automotive Insurance
  • Driver's License
  • Act 34
  • OAPSA
