UPMC Western Behavioral Health Service Coordinator

UPMC Western Behavioral Health is hiring Service Coordinators in the Pittsburgh area! As a Service Coordinator, you will specialize in connecting clients with community resources. Although this role offers a hybrid work model, allowing you to complete administrative tasks either at home or in our office, with minimal need to visit the office, the majority of the work is down in community meetings with clients in their home or other agreed upon locations. Mileage and parking reimbursement are provided. The schedule for this position is 8:30 AM to 5:00 PM, and it is community based.

This position offers a sign-on bonus of up to $3,000!

Responsibilities:

  • Lead coordination of mental health services based on individual needs.
  • Facilitate interdisciplinary planning meetings and ensure service delivery.
  • Maintain contact during hospitalizations and transitions of care.
  • Participate in departmental on-call rotation.

Assessment & Planning

  • Conduct collaborative assessments of individual and family strengths and needs.
  • Develop and implement individualized service plans.
  • Document assessments and plans in compliance with regulations.

Resource Navigation

  • Identify, access, and coordinate internal and external resources.
  • Maintain and update a catalog of community services.
  • Serve as liaison to connect individuals with appropriate supports.

Advocacy & Cultural Competence

  • Advocate for individuals across service systems.
  • Deliver culturally responsive services respecting diverse backgrounds.

Documentation & Compliance

  • Maintain accurate records of service coordination activities.
  • Ensure documentation meets regulatory standards.

Bachelor's Degree in a Human Services field such as Social Work, Psychology or Counseling required. Other fields of study may be considered with coursework of at least 12 credits in Human Services. One year related experience required. Prior experience working in community settings required. Safe driving record required.

Licensure, Certifications, and Clearances:

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Comprehensive Crisis Management (CCMC)
  • Driver's License
  • Act 31 Child Abuse Reporting with renewal
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
