We're looking for a dedicated professional who is passionate about serving the geriatric population, experienced in social services delivery, and skilled in leading and mentoring a team. In this leadership role, you'll help shape a supportive, person-centered environment for our residents and families, while working alongside a committed interdisciplinary team.

What You'll Do:

• Lead and manage all aspects of the Social Services Department.
• Provide support for residents and families including assessments, discharge planning, and care coordination.
• Supervise and mentor staff, promote professional growth, and ensure regulatory compliance.
• Serve as a key liaison between residents, families, staff, and external care partners.
• Uphold excellence in service and communication, helping create a community of dignity and respect.

Requirements:

• Bachelor's degree in Social Work or a related human service field
• Licensed Social Worker (LBSW or LMSW) in Michigan
• At least 1 year of supervised social work experience in healthcare
• 1–2 years in a supervisory role, preferably in long-term or geriatric care
• Knowledge of MDS, discharge planning, and state regulations
