What you will do in this role: Officially we title this job Social Services Director, but in other centers it may be called a Medical Social Worker or Discharge Planner or a PRSD.

As a Social Services Director, you will help to organize programs for our behavioral health population as well as spearheading multidisciplinary services so they can safely transition home. This could involve coordinating communication with therapy, nursing and other medical needs. You will juggle many things at once: One moment you may be ordering DME & meds, then conducting a Psych/Social Assessment and the next you could be involved with educating the family as they prepare to become the primary caretaker.

You will do whatever is needed to support medical & psychological needs of the inpatient guests.

What we need from you:

Bachelor's Degree (or higher) in Social Work; LSW is preferred but not required

Extensive experience with high volume discharge planning

Have experience in long-term care, skilled nursing and/or rehab center

Electronic medical records experience (Point Click Care or PCC is ideal)

We follow the CMS & IDPH rules for the Covid-19 vaccine. You should be fully vaccinated or you will be required to submit an exemption. Details will be discussed during your interview.
