And we're looking for a Case Manager to walk alongside some of the most vulnerable members of our community as they transition from homelessness to long-term, sustainable housing.
About the Role
This is more than a job – it's a frontline role that requires empathy, resilience, and a commitment to person-centred, trauma-informed care. You'll deliver brief interventions, coordinate care plans, and provide assertive outreach to build trust and connect individuals to essential services. Working across sites in St Kilda, Footscray, North Melbourne, and Kensington, you'll be part of the Greenlight Supportive Housing Program – a collaborative effort with VincentCare and Sacred Heart Mission. In this multi-disciplinary team, you'll support people transitioning from chronic homelessness to stable housing through holistic, wrap-around case management.
Reporting to the Greenlight Supportive Housing Team Leader, this is a permanent full-time position, based in North Melbourne, VIC. Compensation is in accordance with SCHADS Award Level 5.
