HIV Psychosocial Support Advocate

Pay: $26.42 per hour

Schedule: Monday – Friday, 9:00am – 5:00pm

Summary/Objective

The Psychosocial Support Advocate provides psychosocial support services to individuals living with HIV/AIDS. This role develops ongoing relationships with clients and supports engagement in medical care and social services. Responsibilities include appointment reminders, accompaniment to appointments, care coordination, and facilitation of support services. The Advocate works closely with Medical Case Management staff to promote continuity of care and improved health outcomes.

Essential Functions

  • Complete client intake and assessments in accordance with program guidelines.
  • Assist in developing and implementing service or care plans.
  • Provide ongoing psychosocial support and maintain regular contact with assigned clients.
  • Accompany clients to appointments including medical, housing, court, and financial services as needed.
  • Prepare clients for interactions with service providers and support them during appointments.
  • Conduct appointment reminder calls and follow-up outreach.
  • Facilitate support groups as assigned.
  • Monitor and track client CD4 and viral load information as required.
  • Maintain accurate and timely documentation in electronic health records and tracking tools.
  • Update client charts and participate in quarterly chart audits.
  • Monitor spreadsheets and electronic systems for upcoming client appointments.
  • Collaborate with Medical Case Management staff on shared caseloads.
  • Assist Medical Case Management staff with coordination and follow-up activities.
  • Participate in Consumer Advisory Board activities as assigned.
  • Develop and maintain relationships with community-based social service providers.
  • Perform other duties as assigned.

Work Environment

  • This position operates primarily in an office environment with occasional client home visits and community-based work.

Physical Demands

  • Duties may require standing, walking, and sitting for extended periods of time.

Position Type/Expected Hours of Work

  • Monday through Friday, 9:00am to 5:00pm.

Required Education and Experience

  • High School Diploma or GED required.
  • Support services or case management experience preferred.
  • Working knowledge of serving individuals living with HIV/AIDS and addiction preferred.
  • Experience working with culturally diverse communities.
  • Familiarity with community-based agencies serving people of color.
  • Bilingual or bicultural candidates strongly encouraged.
