Pharmacist FT

Provides pharmaceutical care services in the form of clinical support and distribution services in coordination with physician and nursing services in delivering quality and cost effective drug therapy and drug management to patients. Supervises technicians and operations in areas assigned by the Director or Supervisor of Pharmacy Services.

Education: B.S. or equivalent in Pharmacy. Specialized knowledge of pharmacokinetics, pharmacodynamics, drug literature review, therapeutics, computer applications, aseptic technique. Qualified to become Arkansas licensed by the Board of Pharmacy. The subject of this continuing education may be at the discretion of the Director of Pharmacy as appropriate to the practice setting of the hospital pharmacy department. One to two years experience in a hospital setting.

Experience: New graduate and up.

Licenses/Certificate: Arkansas Licensed

Essential Job Functions:

Identify and plan for safety precautions to prevent patient injury.

Meet departmental standards and competencies as evidence by department-specific documentation.

Observe professional ethics in maintaining confidential information acquired regarding person, financial, medical or employment of patients, families, and employees of SARH.

Must follow and enforce accepted safety practices for patients and SARH.

Must report hazards and initiate appropriate action.

Must participate in safety instructional programs

Interprets and evaluates physician's orders to prepare, calculate (concentrating on age specific dosing), and dispense appropriate medication dosage forms for inpatient and outpatient treatment in accordance with the New Mexico Board of Pharmacy regulations, federal drug laws, and the Joint Commission on Accreditation of Health Care organizations.

Supervise all assigned personnel and operations of assigned area of responsibility.

Review, maintain, and screen patient medication records and profile for allergies, drug interactions, and therapeutic duplications while entering orders and/or verifying patient medication cassettes.

Communicate with physicians and nurses to insure prompt clarification of orders that are unclear, illegible or incomplete and to recommend alternatives for inappropriate therapy of non-formulary items.

Initiates and maintains liaison service to nursing units and other departments.

Provide drug information and/or education programs to healthcare professionals.

Must be able to assess problems and recommend resolutions to problems to his/her immediate supervisor.

Must also be able to recommend changes in a patient's drug therapy and must be able to interpret and evaluate physician orders for the most cost effective appropriate therapy. e.g., deleting therapeutic duplications, preventing unnecessary extended therapy, converting interactions and drug related allergies and recommending formulary substitutions for non-formulary drug orders. This intervention also will prevent the likelihood of drug induced risk to the patient. The minimum number of annual interventions shall be at least 365 or a number determined by the DOPS as a benchmark for the entire staff.

Must be able to calculate total parenteral nutrition orders and other intravenous therapies where attention to detail is crucial to prevent injury or death.

Makes routine reports to the Director as deemed necessary by the Director. This position may be asked to help gather data for the CQI and QA process.

Staff Pharmacist may be asked to rotate between on an as needed basis to cover vacations, etc.

This position will serve on certain hospital wide committees as deemed necessary by the Director of Pharmacy.

Responsible for training new employees that are assigned to his/her supervision.

Performs other duties as assigned.

Produces at least the minimum number of interventions on a monthly basis.

Performs additional tasks as assigned.
