Job Responsibilities:

  1. Manage and support TSP Case Managers, Case Management Associate, Life Skills Therapist, Vocational Counselors, Peer Liaisons, and ShepherdCare Case Managers.
  2. Collaborate with staff to address patient issues (medical and emotional concerns, health and safety goals, vocational interests/goals, and patient financial status) as they arise.
  3. Ensure that staff and clients develop a mutually acceptable plan of care to maximize the potential for a successful community reintegration and a meaningful life post injury.
  4. Provide direct service to clients/others when needed, as staff are unavailable or need additional support.
  5. Provide direct assistance with customer service issues, ensure suitable solutions.
  6. Develop and provide staff with educational opportunities to ensure clients and caregivers receive safe and quality care, always with a focus on outcome measures and standard of care practices.
  7. Work with data analytics to collect clinical outcomes data, patient satisfaction results, and perform analyses in support of process improvement initiatives. Report to SMT, physicians, and CM Council on TSP outcomes and rehospitalization data.
  8. Collaborate with medical staff to create action plans to reduce ER and rehospitalization rates post discharge.
  9. Provide in-services and presentations, internally and externally.
  10. Support advocacy for patients and their families by promoting high standards of patient care, participating in program evaluation and development and other designated department related tasks as identified.
  11. Conduct periodic chart reviews, prepares a report of findings and recommendations, develop a plan for corrective action, as needed.
  12. Maintain policies and procedures in compliance with JCAHO/CARF standards.
  13. Lead engaging staff meetings, regular rounding opportunities, and yearly evaluations of staff.
  14. Performs utilization review on all <30 day readmissions and reports findings to UM Committee.
  15. Practices proper safety techniques in accordance with Center and departmental policies and procedures. Responsible for the reporting of employee/patient/visitor injuries or accidents, or other safety issues to the supervisor and in the occurrence notification system.
  16. Monitors and ensures compliance with all regulatory requirements, organizational standards, and policies and procedures related to area of responsibility. Identifies potential risk areas within area of responsibility and supports problem resolution process.

Required Minimum Education:

  • Bachelor of Science in Nursing

Required Minimum Certification:

  • No professional certifications required.

Required Minimum Experience:

  • 5 years' related experience.
  • 2 years' rehabilitation experience.

Required Minimum Skills:

  • Demonstrated leadership abilities
  • Completes orientation and initial assessment of competency successfully by determined due date.
  • Completes mandatory education and annual competency assessment requirements by determined due date.

Preferred Qualifications:

  • 2 years' supervisory experience preferred
  • CRRN, CCM or CMC certification, preferred.

Physical Demands:

  • Uses body, tools, or special devices to safely, effectively, and efficiently to move, guide, or place patients. Involves some latitude for judgement with regard to precision attained and the selection of what adaptive device(s) to utilize.
  • Ability to transfer and position patients, consistent with lifting up to 50 lbs.

Working Conditions:

  • Some potential for exposure to blood and body fluids.
