This is a part-time position (24 hours/week). Social Services coordinator position identifies the needs of each resident/patient/families and assures that the needs are met by the appropriate discipline. Assists patients/residents/families in improving/maintaining and their ability to manage their physical, mental and psychosocial needs. Acts as the grievance officer for the facility.

BSW or BA in sociology/social work or related human services field
Minimum of one-year experience in a long term care setting
Excellent oral and written communication skills
Knowledge of community resources
Experience working as part of an interdisciplinary team
Computer experience
