Social Service Assistant

Newport Meadows Nursing and Rehabilitation Center is searching for an experienced Social Service Assistant to join our Long-Term Care team! Long term care experience with the geriatric population is required for this position. Job Type: Part time 24hrs a week

Job Responsibilities

  • Orients resident and family to nursing center, helping both adjust to new environment, and offers support as needed
  • Completes social history and MDS assessments, and develops care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken
  • Conducts/participates in resident care conferences with resident, family, and appropriate staff
  • Advocate and promote resident rights
  • Receives complaints from residents and families, forwarding complaints to appropriate staff to ensure resolution. Documents complaints. Follows facility policies and procedures
  • Performs other related duties, as assigned

Education & Qualifications

  • High-school diploma or GED required.
  • 1 year Social Services experience in long-term care is required
  • A physical examination and/or evidence from a physician indicating freedom of communicable disease to be available upon employment.
  • Must be in good physical and mental health.
  • Must be knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care.
  • Must possess the ability to deal tactfully with personnel, resident, family members, visitors, government agencies/personnel, and the general public.
  • Knowledge of elderly, ill and/or disabled.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Ability to work independently or part of a group.
  • Effectively communicate with others.
