✨ About The Role
- Engage and enroll individuals virtually and in-person into the program, focusing on those living with Serious Mental Illness/Substance Use Disorder, experiencing homelessness, or with high medical needs
- Provide education to partners about Pair Team's services and enroll eligible members in the Enhanced Care Management program
- Manage the end-to-end enrollment process, ensuring timely communication and updates on progress
- Utilize technology tools for effective enrollment funnel management and meet key metrics to support the mission of improving underserved communities' wellbeing
- Requires physical presence in Santa Cruz, California 1-2 times a month, with the rest of the role being remote-based

- Experience working in a metric or quota-driven environment, dedicated to community improvement and exceeding performance indicators
- Strong communication skills, bilingual in English and Spanish, with the ability to build trust and engage with individuals in underserved communities
- Comfortable with technology platforms and adaptable to an ever-changing environment focused on increasing access to high-quality care
- Willingness to obtain a Community Health Worker certification, if not already certified
- Results-driven, with a track record of successful engagement campaigns and a commitment to improving the wellbeing of underserved communities
