We are searching for a compassionate and experienced Social Services Director to lead our social services programs. In this full-time, salaried role, you will be a resident advocate, ensuring their emotional and social needs are met with professionalism and care.

Responsibilities:
Lead the planning and implementation of resident-focused social services.
Offer emotional support and counseling to residents and their families.
Manage and coordinate the discharge planning process with the care team and external agencies.
Schedule and arrange all necessary on-site and off-site resident services.
Oversee practical needs, including personal shopping and resident belongings management.
Maintain effective, empathetic communication with all stakeholders.
Qualifications:
Bachelor's degree in Social Work or a related field (MSW a plus).
2+ years of supervised social work experience in a healthcare setting.
Ability to handle complex, confidential situations.
Strong organizational and communication skills.
Proactive and persistent approach to problem-solving.
