Victorian Post Acute is seeking a compassionate and experienced Social Services Director to lead our social services programs. In this full-time, salaried position, you will serve as a resident advocate, ensuring that their emotional and social needs are met with professionalism and care.

Responsibilities:

• Lead the planning and implementation of resident-focused social services programs.
• Offer emotional support and counseling to residents and their families.
• Manage and coordinate the discharge planning process in collaboration with the care team and external agencies.
• Schedule and arrange all necessary on-site and off-site resident services.
• Oversee residents' practical needs, including personal shopping and management of resident belongings.
• Maintain effective and empathetic communication with all stakeholders involved in resident care.

Qualifications:

• Bachelor's degree in Social Work or a related field (MSW preferred).
• 2+ years of supervised social work experience in a healthcare setting.
• Ability to handle complex and confidential situations with discretion.
• Strong organizational and communication skills.
• Proactive and persistent approach to problem-solving.
