Social Services Assistant

Salinas Valley Post Acute is seeking a Social Services Assistant to join the team full time. Starting Rate: $23 per hour.

As a Social Services Assistant, you will play a vital role in supporting the emotional, social, and physical well-being of residents under the direction of the Social Service Director. This position requires a compassionate, organized, and resourceful individual to help manage and deliver social services programs and resources.

Key Responsibilities:

  • Resident and family support:
    • Assess residents' social and emotional needs and assist them in adjusting to their institutional life.
    • Help residents achieve the highest possible level of independence and self-care.
    • Provide emotional support to residents and their families coping with illness, stress, and anxiety.
  • Administrative and care coordination:
    • Maintain detailed and regular social service progress notes.
    • Assist with discharge planning, including coordinating with outside agencies and arranging for necessary equipment and services.
    • Schedule on-site and off-site patient appointments for services like optometry, podiatry, and dentistry.
    • Perform administrative duties, such as completing forms and reports.
  • Resource and program management:
    • Inform residents and families about available financial programs, including Medicare and Medicaid.
    • Connect residents with community health and social welfare resources.
    • Assist in developing and implementing social service policies and procedures.
    • Participate in community planning and department meetings.

Qualifications:

  • Education: A high school diploma or GED is required. An Associate or Bachelor's degree is preferred.

  • Experience: Prior experience in a healthcare setting is preferred.

  • Skills:

    • Strong written and verbal communication abilities.
    • Ability to interpret documents and regulations.
    • Excellent problem-solving and reasoning skills.
    • Proficiency with standard office equipment and computer use.
