Job Title: Stimulant Reduction Support Case Manager

Salary Range: $21.50 - $22.79 per hour. Bilingual candidates are eligible for an additional $1.50 Bilingual Differential. This Bilingual Differential is not included in the salary range above.

Position Summary: The Stimulant Reduction Support Case Manager works with participants one-on-one to support reduction in stimulant use by utilizing Contingency Management and coordinating access to behavioral health Cognitive Behavioral Therapy counseling. The role of the Stimulant Reduction Support Case Manager includes following up on referrals, conducting participant intake, facilitating appointments, administering interventions, providing incentives, documentation, sending referrals to other services, and ensuring participant compliance and adherence with program requirements.

Essential Functions and Responsibilities:

• Receive and respond to referral requests
• Conduct individual assessments to assist clients in developing a care plan
• Provide case management services including coordinating access to Cognitive Behavioral Therapy
• Implement contingency management intervention including administration of urinalysis tests
• Provide direct one-on-one coordination services and evaluate participant performance
• Distribute and maintain log of incentives and prize basket items
• Maintain and build client caseload
• Maintain up-to-date information regarding the program
• Maintain up-to-date and accurate client records
• Participate in required training pertaining to program
• Work with Harm Reduction Intervention Manager to ensure proper implementation of program and required data collection
• Participate in clinical supervision and receive support from program manager and relevant staff
• Maintain sensitive and critical documentation
• Other responsibilities as assigned

Ability to work well with others, handle stress appropriately, have reliable attendance

Required Qualifications:

• TB test (provided at agency)
• Ability to pass agency criminal background check
• Minimum 1 year of experience providing direct client support or case management experience, or equivalent combination of relevant and/or related experience and education
• Minimum 1 year of experience with chart notes, file maintenance, data entry
• Ability to stay calm, kind, non-judgmental with a variety of vulnerable clients
• Ability to maintain professionalism, which maintains a safe, respectful, and productive environment for clients, volunteers and staff
• Ability to work flexible hours including working within availability of office spaces
• Ability to work in busy, dynamic, changing environment
• Desire to work with diverse populations
