This job introduces the full scope of services offered to clinical staff. Ensures the ongoing updates and education for referral sources. Responsible for the successful clinical coordination of care from the referral source into the home setting. Provides and utilizes clinical expertise and skills to assist referral sources and/or patients with: identification of appropriate candidates for home infusion or home health care, identification of the patients or caregivers ability to provide care, patient education and training regarding therapy and supplies. Assumes responsibility for clarifying expectations of care in the home, medication connection at the hospital/facility in order to expedite the discharge into the home setting as needed.

Education:
Required – Registered Nurse Diploma
Preferred – Bachelor’s degree in Nursing
Work Experience:
Required – 3 years of Nursing experience
Certifications:
Required – Current RN License in the state of practice
Basic Life Support (BLS) from the American Heart Association
