As an Early Intervention Service Coordinator, duties include working with families and providers to develop Functional Individualized Family Service Plans (IFSPs), working with multidisciplinary teams to determine families' needs for support and services, conduct home visits, coordinate and facilitate services and transition planning; and participate in the authorization and monitoring of early intervention services.

Minimum Education and Experience: Bachelor's degree in Special Education, Elementary Education, or Early Childhood Education, Psychology from an appropriately accredited institution, and two years of professional experience working with the population served; or Bachelor's degree in the human services field from an appropriately accredited institution and three years of professional experience working with the population served; or an equivalent combination of education and experience.
