Schedule: Monday - Friday 8:00 AM - 5:00 PM; Full-Time Department: Care Navigation - supporting heart failure patients Location: Presbyterian Medical Center - Charlotte, NC; Presbyterian is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte. This role is ideal for an experienced RN looking to serve as the primary liaison between the patient and his/her medical team and be a patient advocate.

What You'll Do Support heart failure patients on any unit at PMC Find resources for patients including food, medicine, and clinics Work to discover what is causing the patient to return to the hospital Review medication and diet adherence issues with the patient Provide some education on the patient's diagnosis Partner with case management and palliative care teams Work towards goal of reducing readmission and ER visits
What We're Looking For Education: 4 Year / Bachelors Degree, required. Experience: Three to five years of nursing experience, required. Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred. Licensure/Certification: RN, required. Additional Skills/Requirements (required): Ability to successfully complete generic and department-specific skills validation and competency testing.
