The Support Coordinator is dedicated to assisting NDIS participants in the coordination and implementation of all supports outlined in their funded plans. This includes informal, mainstream, community, and NDIS-funded supports.
This is a full-time permanent role that may require travel across the Hunter region to meet with participants and their families as required.
The primary objectives of this role are:
You are an experienced Support Coordinator who thrives on delivering person-centred, innovative supports that empower participants to achieve their goals. You excel at managing a diverse portfolio, planning work effectively, and building strong relationships with participants and their families. Your knowledge of NDIS frameworks and pricing guidelines ensures compliance while driving positive outcomes.
