Let's Talk About This Role

The Support Coordinator is dedicated to assisting NDIS participants in the coordination and implementation of all supports outlined in their funded plans. This includes informal, mainstream, community, and NDIS-funded supports.

This is a full-time permanent role that may require travel across the Hunter region to meet with participants and their families as required.

The primary objectives of this role are:

  • Strengthen and enhance the participant's ability to coordinate supports, participate in the community, and ensure the delivery of their plan within the allocated budget.
  • Build the participant's capacity to achieve greater independence and self-direct their services and supports in the long term.
  • Provide the participant and the NDIA with timely reports on outcomes and success indicators.
  • Develop and implement solutions and strategies that enhance the participant's inclusion in their community.

You are an experienced Support Coordinator who thrives on delivering person-centred, innovative supports that empower participants to achieve their goals. You excel at managing a diverse portfolio, planning work effectively, and building strong relationships with participants and their families. Your knowledge of NDIS frameworks and pricing guidelines ensures compliance while driving positive outcomes.

The key outcomes of this role:

  • Participants consistently report positive experiences and feel supported.
  • Delivery of innovative, person-centred supports tailored to individual needs.
  • Develop and implement solutions and strategies that enhance participants' inclusion in their community and build confidence to self-direct by engaging with their services and networks.

Your essential skills and knowledge:

  • Passionate about empowering people with disabilities and fostering dignity, respect, and inclusion.
  • Proven experience in portfolio management and effective work planning.
  • Excellent communication and engagement skills with participants, families, and stakeholders.
  • Strong written and computer skills for case noting, CRM maintenance, and report writing.
  • In-depth knowledge of NDIS, the Disability Services Act, and NDIA's Pricing Arrangements and Price Limits Guide.
