The role of On-Call Transition Services Program (TSP) Advanced Practice Clinician (On-Call TSP APC) is to provide comprehensive nursing care and treatment for justice involved individuals within Multnomah County. The On-Call TSP APC will be under the clinical supervision of the Corrections Health Medical Director and will function as the main medical provider for the Transition team; guiding clinical care with a focus on medication supported recovery and harm reduction, and collaboration with Health Department and community providers.

As an On-Call TSP APC Health, you will help serve in The Multnomah County Inverness Jail and Multnomah County Detention Center. As the On-Call TSP APC, other duties will include:

  • Diagnose and treat acute and chronic medical conditions according to scope of practice. This would include: conducting a medical history, conducting physical exams, ordering and interpreting diagnostic tests, devising a course of treatment including Medication Supported Recovery (MSR), discussing alternative treatment; the risk and benefits of treatment, prescribing and monitoring medications, including MSR, appropriate counseling of patients and reassessment of the patient at appropriate intervals. Documenting appropriately in EHR system
  • Provide consultation to nursing and other Corrections Health staff as indicated for continuity of client care. Provide in-service to staff as a group or individually, as requested and precept students when assigned. Assist in the development and implementation of protocols, procedures and other teaching tools. Attend provider meetings as scheduled and morning shift report. Contribute to patient care planning and identification of patient needs.
  • On-call for after-hours provider coverage as scheduled.

To Qualify:

We will consider any combination of relevant work experience, volunteering, education, and transferable skills as qualifying unless an item or section is labeled required. Minimum Qualifications/Transferable Skills*:

  • Master's Degree in Nursing AND one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.

Certification Requirements:

  • License required to practice medicine as a Nurse Practitioner as outlined by Oregon Revised Statutes (ORS) 678.375 OR License required to practice medicine as a Physician Assistant as outlined by Oregon Revised Statutes (ORS) 677.515
  • CPR certification required
  • DEA License

Other: Passing a Multnomah County Sheriff's Office background check is required. This position requires a background investigation, which may include being fingerprinted. Employment is contingent on passing a background investigation. The employee holding this position is designated as an Essential Employee under Multnomah County Personnel Rule 3-15-015, Essential Employees.   • Experience working in a correctional facility.
  • Experience in Addictions Medicine and Medication Supported Recovery.
  • Training or experience in working with Electronic Health Record systems.
  • Experience working with targeted groups and with ethnically and culturally diverse populations.
