Under the leadership of the Post Acute Services department director, assistant director, Nurse Manager and Clinical Supervisors, the LVN performs patient care within the scope of the Vocational Nurse Practice Act. The LVN provides patient care to policies and procedures of CHOMP, Westland House, Inpatient Rehab Unit and Hospice of the Central Coast as applicable. The LVN participates in patient and family teaching, works collaboratively with the patient care team, and uses resources efficiently. The LVN recognizes the importance of meeting customer expectations, demonstrating a positive caring attitude, and performs the duties necessary to fulfill the customer's needs. Excellent organizational skills, the ability to multi-task, and good critical thinking skills are important dimensions of this position.
Previous skilled nursing, acute care, or home health experience is preferred.
Level of education sufficient to obtain CA LVN license.
State of California LVN license required. American Heart Association Healthcare Provider BLS required.
Assigned Work Hours: Varied shifts, varied days. Will work in both Westland House and Inpatient Rehab Unit. Must be willing to work one weekend per pay period.

Pay Range (based on years of applicable experience): $47.19 to $63.13
The hours employees work determine when a shift differential is paid.
Hourly Evening Shift Differential: $2.76
Hourly Night Shift Differential: $4.13
