Fee For Service Social Worker

Job Title

This position will cover Bronx and Manhattan regions.

Fee for Service Visit Rates: $65 per visit.

Orientation Daily Rates: $165 per day.

Responsibilities

Assesses patient psycho-social issues to ensure adequate delivery of home care services in order to meet agency goals and required objectives. Obtains psycho-social history and conducts evaluation and assessment of patient including adequacy of patient support system. Provides supportive counseling in regards to emotional, psychological and concrete needs including discharge planning. Interprets and provides information about available benefits, entitlements and community programs to patients and family. Counsels patients and recommends community based resources regarding their psycho-social needs.

Qualifications

• Master's degree (MSW) plus New York State License required.
• Knowledge of the operations of community-based health and social services programs, concrete services and government entitlements required.
• Must possess good oral and written communication skills along with excellent interpersonal skills.
• Home Care experience preferred.
• Must be able to provide own transportation in order to make visits in assigned area(s) within greater NYC.
