We currently have one Registered Nurse Social Services position in the Access, Aging, and Disability (AA&D) Division of Community Services on the Reassessment team. This position will be providing annual MnCHOICES Assessment services to people with varying abilities and ages. Services are provided to a broad population using an inclusive, person-centered approach that includes race equity. Qualified candidates with diverse perspectives are encouraged to apply. One or more years of experience in Home and Community-Based Services (HCBS) is preferred for MnCHOICES Certification. DESIRABLE EXPERIENCE:

HOURS/DAYS: 8:00 a.m.-4:30 p.m., Monday-Friday based out of the Stillwater Office. Work schedules (hours and days) and location are subject to change based on department needs. The position offers teleworking and flexible hours that align with the business need.

SELECTION TECHNIQUE: Your training and experience will be rated based on the required application material submitted and the top scoring candidates will be forwarded to the hiring department for further consideration. ANTICIPATED HIRING RATE: $72,675.20 - $99,153.60 Annually

JOB SUMMARY: The Registered Nurse Social Services determines client eligibility, provides skilled nursing for assessments, and manages cases and care coordination for the waiver programs by implementing plans to protect vulnerable people and promote independence.

EMPLOYMENT CRITERIA MINIMUM QUALIFICATIONS: This position requires a two-year degree in nursing from an accredited nursing school, AND a Minnesota Registered Nurse License.

DUTIES & RESPONSIBILITIES:

  • Provide nursing for intake and assessment:
    • Conduct assessments to determine the nursing needs of clients (physical, cognitive, mental, family, social, and community support perspectives);
    • Determine resources and referrals to community-based care and services; and
    • Make referrals to medical and community support providers.
  • Provide program delivery:
    • Perform case management and care coordination;
    • Monitor the quality of services, identify service gaps, and ensure services are delivered within authorized budget;
    • Provide health care education and resources to clients to resolve or prevent health care problems;
    • Provide consultation on complex medical issues;
    • Develop, write, and implement community support plans to address identified problems;
    • Develop and implement abuse prevention plans;
    • Provide case testimony or written recommendations in county or district court or in administrative appeals hearings;
    • Advise clients of possible resources, rights and responsibilities under programs or services;
    • Intervene in crisis situations;
    • Coordinate and secure emergency services;
    • Negotiate and resolve conflict situations;
    • Coordinate and assist clients with finding alternative living arrangements;
    • Coordinate and deliver program information to the general public, special interest groups and professional organizations;
    • Collaborate and consult with others including professional staff and community agencies regarding clients, community services or programs.
  • Perform other duties as assigned.

WORK ENVIRONMENT: Semi-sedentary involving office and outside field work. Lifting requirement may include lifting up to 20 pounds. Exposures may include but not be limited to disagreeable odors, communicable diseases, bodily fluids and behavior challenges. Use of general office equipment to include telephone, cell phone, pager, personal computer, laptop computer, electronic recorder, calculator, etc. Additional physical/mental requirements that occur may not be listed above, but are inherent in performing the position's essential functions.
