Community First Services and Supports (CFSS) is a new program replacing the Personal Care and Consumer Support Grant program in Minnesota. CFSS is being implemented through a rolling launch as individuals are assessed and reassessed for eligibility. A central feature of the program is the provision of Consultation Services, which offer non-biased support to help individuals make informed decisions about how to use their CFSS allocation and develop individualized Service Delivery Plans (SDPs). This position plays a key supportive role in strengthening the capacity of Consultation Service Providers by offering technical assistance and guidance in the development of individualized SDPs. When plans do not align with CFSS policy, the incumbent assists providers in understanding and applying the necessary adjustments. The work is carried out in close collaboration with the CFSS implementation team and other divisions across the state, including Disability Services Division, Provider Enrollment and Eligibility, Aging and Adult Services, MNIT, Consultation Services providers, and individuals receiving services. Through this coordinated effort, the position helps facilitate timely and successful transitions from legacy programs into CFSS.
1 year professional experience with home and community based services demonstrating the following:
OR 3 year advanced paraprofessional experience with home and community based services demonstrating the following:
Knowledge of the MnCHOICES application and where to find assessment data to ensure SDPs meet the assessed need of the person and fit within the requirements of CFSS program
Experience communicating complex policy information and instructions
Knows HIPAA requirements in relation to personal health Information (PHI) and associated data access, sharing and security concerns
Thorough understanding of collective bargaining agreements
Variety of experiences working effectively with others from different backgrounds and/or cultures
