Social Services Designer

The Social Services Designer is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for resident/guest discharge planning.

Responsibilities

• Meeting the resident/guest on or shortly after the admission and following them until they discharge to the community.
• Attend the 72 hour care plan meeting.
• Ordering equipment for the community setting.
• Arranging home health nursing for after discharge, as needed.
• Coordinating the medications for community use.
• Coordinate the entire discharge, with IDT involvement.
• Attending care conferences for sub-acute residents/guests.
• Attending Medicare meetings.
• Assist in procuring any item related to the discharge process for residents/guests anticipated to be returning to the community.
• Initiate and coordinate the completion of the Post Discharge Plan of Care.
• Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community.

Qualifications

High school diploma/GED
