Candidate will have a strong background in Social Work or RN Case management and care coordination. This position participates in care management and ensures resident needs are met through social service programming, making appropriate referrals to outside agencies and community resources. This position supports resident's social, emotional, and psychological needs, identifies ways to support resident preferences, routines, and concerns.
Coordinate resident care plan conferences including, initial, quarterly, change of conditions, special needs, and discharge conferences.
Develop and maintain person-centered care plans for each individual resident.
Provide discharge planning and after services.
Participates with resident admissions, transfers, and discharges inclusive of all required documentation and charting.
Completes all necessary federal, state, local, and facility policies and procedures. As well as assisting residents/responsible party with financial and legal matters such as guardianships/conservatorship, etc.
Acts as a facility resource to provide intervention with difficult resident and family issues, utilizing grievance and conflict resolution skills.
Preferred: Licensed Social Work Certificate BSW or Registered Nurse in the State of Michigan.
Demonstrated excellent oral and written communication skills.
MDS and short-term discharge planning experience.
Minimum 1 year case work.
Minimum 1 year work with elderly/geriatric population.
