Patient Navigator

The Patient Navigator is responsible for guiding patients through the complexities of the healthcare system. Working collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for all assigned patients, the Patient Navigator ensures that safety, best practice, and high quality standards of care are maintained across the continuum. This role provides support and resources to patients, including connecting patients to available services, navigates insurance authorizations, coordinates care between providers, and works with healthcare providers to ensure that patients are receiving the best possible care.

Responsibilities:

  • Extract data from medical record sources to gather information and perform outreach.
  • Communicates details to the appropriate team, providers, and/or supervisors.
  • May assist with obtaining authorizations and scheduling where appropriate.
  • Works closely with physicians and the multidisciplinary care management teams.
  • Facilitates care coordination, and documents efforts.
  • Utilizes the appropriate reports to prioritize outreach to ensure care coordination.
  • Coordinates a wide range of activities and education to support the patient population by utilizing a variety of communication methods including but not limited to telehealth, face-to-face, text, telephone, or email.

Education and Certification Requirements:

  • High School Diploma or Equivalent (Required)
  • BLS American Heart E-Card (BLS AHA ECARD) - American Heart Association (AMERICAN HEART)
  • BLS Cert American Heart_non ecard (BLS AHA) - American Heart Association (AMERICAN HEART)
  • BLS Certification Grace (BLS GRACE) - Employee Grace Period for Essential Credential (GRACE)
  • BLS Cert Red Cross (BLS RC) - Red Cross (RED CROSS)

Additional Job Information:

  • Complexity of Work: Success will be measured by the results of the process and outcome performance measures of the population of patients. Requires critical thinking skills, effective communications skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Knowledgeable in Excel, Word and PowerPoint and able to use electronic medical records such as EPIC.
  • Required Work Experience: One (1) year of experience in a healthcare setting required.
