The objective of the Supportive Services Case Manager is to provide wrap around/supportive case management services to increase aid & follow-through with individuals who are currently experiencing housing instability. The case manager works closely with the intake team to identify & support individuals needing more immediate or urgent supports to maintain their current level of housing &/or reduce the risk of homelessness. This is a collaborative role that will work directly with individuals & support agencies to help stabilize housing crisis situations & secure/maintain housing. Good interagency collaboration & communication skills are a must. This role operates primary out of the 255 Independence Drive Office & has some remote hours as needed.
