Supportive Services Case Manager

The objective of the Supportive Services Case Manager is to provide wrap around/supportive case management services to increase aid & follow-through with individuals who are currently experiencing housing instability. The case manager works closely with the intake team to identify & support individuals needing more immediate or urgent supports to maintain their current level of housing &/or reduce the risk of homelessness. This is a collaborative role that will work directly with individuals & support agencies to help stabilize housing crisis situations & secure/maintain housing. Good interagency collaboration & communication skills are a must. This role operates primary out of the 255 Independence Drive Office & has some remote hours as needed.

Essential Functions:

  • Coordinates & participates with HAC’s Intake team to identify at-risk individuals who may need more intensive case management services to remain stably housed
  • Completes comprehensive assessments related to housing needs & supports client in access services in accordance with housing goals
  • Collaborate with other departments & agency programs to maximize participant outcomes, program goals, & agency mission
  • Collaboratively work with funding resources to find & secure prevention funds for eligible participants if needed
  • Provides case management services that potentially includes: providing appropriate resources, tools, & counseling to assist participants in preventing homelessness; Make referrals for services that would increase housing stability including, but not limited to, primary care, behavioral health, substance abuse, & legal services; following up to ensure that the referrals were completed; tracking & documenting participant progress
  • Acts as a liaison & advocate with landlords to mitigate issues/advocate for participants as directed
  • Collaborate & communicate closely with other agencies/programs also involved in participant care (Community Services, Police Department, DMH, Domestic Violence Services, Legal Services, etc.)
  • Document & maintain up-to-date information on services provided to participants in the electronic tracking system & hard copy case records
  • Communicate regularly with supervisor & participate in supervision sessions.

Additional Duties:

  • Attend all meetings, trainings, & workshops as required by Director.
  • Other duties as assigned.

Required Qualifications, Education, & Experience:

  • Lived experience with homelessness prior to age 25.
  • Valid MA Driver’s License

Preferred Qualifications, Education, & Experience:

  • Bachelor’s Degree
  • Two years’ experience with similar participants &/or housing case management experience

Hours of Work:

This role is an hourly, non-exempt position. This is a 35 hour/week position (70 hours biweekly)
