Provides staffing and workforce solutions for Massachusetts state agencies, supporting public services through recruitment, placement, and human resources management.
Ability to work collaboratively with alcohol/drug dependent clients from diverse legal, social, economic, ethnic, cultural and racial backgrounds as well as family members, insurance providers and outside providers
Possess the ability to maintain healthy boundaries
Knowledge of appropriate confidentiality policies and procedures
Ability to work independently without direct supervision while adhering to the program’s philosophical team approach
Strong and effective verbal, written and organizational skills
Knowledge of basic computer skills and software programs including the ability to learn, use and train HPTC’s electronic medical record
Recovery Support Navigator Responsibilities:
Compliance with Federal Regulations, HIPAA, 42 CFR Part 2, Confidentiality of Alcohol and Drug Abuse Patients
Compliance to and working knowledge of all applicable HPTC policies and program procedures including but not limited to those relating to ethics/boundaries and duties of role
Maintain safety and security of client/staff at all times
Ensure timely contact of Program Director/Authorities in event of a crisis/emergency
Attend all mandatory trainings, staff meetings, and supervision meetings as required
Provide client-centered case management CSP services as indicated in needs assessment/service plan (transportation, 12 step meetings, linkages to medical/counseling appointments, court etc.)
Provide coordination with HPTC outpatient units in region for client education/approach/sign-ups for CSP program, following Initiation policy
Coordinate monthly regional group activity
Provide 24 hour on-call coverage in program rotation
Utilize Motivational Interviewing and Recovery Coaching in all client encounters
Maintain appropriate level of productivity/billable hours
Collaborate with outside providers as indicated
Obtain insurance authorizations and requests for additional units in required time frame
Accurately complete/document needs assessments, service plans, encounter notes, and service delivery reports in required time
Accurately complete all required billing sheets and expense reports in required time frame
Accurately document all client progress on required forms in required time frame
Accurately document and submit weekly schedule in required time frame
Investigate and develop community contacts and cooperative networks with other community groups and agencies
Coordinate with other departments and facilities to enhance and ensure quality care for all clients
Cooperate with other health and social service agencies in an effort to increase community services
Other duties as assigned by the Program Director
Community Based Recovery Services Coordinator Qualifications:
Bachelor’s degree or lived experience of substance abuse and recovery
Valid driver’s license and ownership of a car. Preference given to those with at least one year of related job experience
