Intensive Flexible Family Supports (Iffs) Case Manager

Starting rate $27.25/hour or $27.75/hour if bilingual Spanish or Portuguese speaking. The IFFS Case Manager assists in the development and coordination of focused, intensive supports in response to identified areas of need and difficulty while helping to build family capacity to support the individual child/young adult at home through referral, advocacy, and flexible funding allocations, as needed. This program is based out of Worcester and provides services across Central Massachusetts. Hours are flexible based on the needs of the families served. The anticipated schedule is Monday - Friday 9am-3pm with occasional flexibility to meet the needs of families, Department of Developmental Services (DDS), and outreach events.

Minimum Education Required: High School Diploma/GED

Responsibilities

  • Conduct comprehensive assessment of individual and family, including home visit, using referral and intake tools to determine and identify areas of need.
  • Develop an initial Family Support Plan (FSP) with each assigned family and renew annually.
  • Provide individually tailored support and coordination to families to ensure that goals, as identified in FSP, are met.
  • Broker services on behalf of all individuals and their families.
  • Coordinate and/or secure referrals for all necessary services.
  • Collaborate with DDS to ensure appropriate services for families.
  • Provide ongoing progress reports to DDS on goals.
  • Transport individuals supported when and where needed, if necessary.
  • Report cases of reasonable suspicion of abuse to Department of Children & Families (DCF) and Disabled Persons Protection Commission (DPPC) as outlined in Mandated Reporter laws.
  • Attend and participate in DDS Quarterly review meetings.
  • Attend trainings as assigned; maintain necessary certifications (CPR, First Aid, Safety Care).

Qualifications

  • Bachelor's degree in education, Psychology, Human Services or related field preferred. High School Diploma or equivalent and 3-5 years of experience.
  • 1-2 years' experience in developmental disabilities field.
  • 1-2 years' experience and demonstrated success working with families, preferred.
  • Family members (or guardians) of individuals served by the DDS or DMH are strongly encouraged to apply for this position.
  • Knowledge of family supports and community resources including behavior management resources and plans and/or demonstrated commitment to learn these resources.
  • Demonstrated knowledge of IEP's and school related entitlements and/or demonstrated commitment to learn the navigation process.
  • Ability to work independently.
  • Must have valid driver's license, ability to travel across the Central MA Region, car insurance, and use of a reliable car is required.
  • Basic computer knowledge.
  • High energy level, superior interpersonal skills and ability to function in a team atmosphere.
  • Must have valid driver's license, car insurance, and reliable automobile.
