Community Services Outreach Coordinator

This job posting will be open for not less than two (2) weeks, and if needed, will remain open until a finalist has been selected.

General Description: The position is responsible for diversified human service and outreach duties for the elderly population of the town including recognizing and identifying elders in need of assistance, referring individuals to various programs, assisting with applications for services, providing intake services for Council on Aging assistance programs, and coordinating the emergency food pantry at the Council on Aging.

Primary Essential Functions:

  • Identify homebound elders via communication with Police, Fire, and Health Department, home visits, family, friends, agencies, and other sources.
  • Coordinate off-site informational sessions to educate the community on various programs the Senior Center has to offer in an effort to reduce social isolation.
  • Coordinate and assist elders with involvement and enrollment in various services including Medicare, Social Security, GSSSI and Mass Health.
  • Provide outreach and assist with applications for fuel assistance, food stamps (SNAP), tax abatements, energy credit, farmer\'s market coupons, and other programs.
  • Continue SNAP Outreach Partnership with the DTA and UMMS.
  • Refer and follow-up with various providers of services including Medicare, Mass Health, and SHINE.
  • Perform home visit and complete intake for Meals on Wheels program.
  • Assist in the resolution of crises including referrals to protective services, nurses, and other sources of assistance.
  • Maintain Senior Watch List.
  • Organize, Facilitate and Update list on a monthly basis with multiple disciplines to include; Fire, Police, Health Departments, GSSSI, Housing Authority, Sheriff\'s Dept., and mental health clinician.
  • Educate and Assist elders with senior housing options and Applications.
  • Coordinate Lock Box program with the Police Department and assist individuals with the application process.
  • Attend various meetings and conferences and communicate with other Councils on Aging for the purpose of keeping abreast of senior issues and concerns.
  • Assist to identify areas of need and opportunities for educational programming, as well in organizing and scheduling the events.
  • Coordinate pantry including determining eligibility, overseeing ordering items, collecting and reporting data, overseeing volunteers, and instituting new practices.

Education and Experience: Education- Bachelor\'s degree required, social work field preferred. Certificates, Licenses, and Registrations- Must possess valid Driver\'s License. Experience- Five (5) to Seven (7) years of experience in a similar position, Municipal/COA experience preferred.

Knowledge:

  • Knowledge of Elder Service Assistance programs on the local, state, and federal level.
  • Knowledge of Medicare and Social Security systems.

Ability:

  • Ability to establish and maintain courteous, effective working relationships with co-workers, as well as with program participants and volunteers.
  • Ability to lead small and large groups of participants and volunteers.
  • Ability to respond to crisis situations with clear and critical decision making abilities.
  • Ability to manage time in a busy office setting with many interruptions.
  • Ability to work both independently and as part of a team.

Skills:

  • Skilled in principles and practices of business correspondence and report writing.
  • Strong communication and customer service skills.
  • Strong organizational skills.
  • Proficient in MS Word and Google Workspace.
