Senior Children And Youth Services Liaison

This position works collaboratively with child serving state agencies ensuring members receive care coordination and appropriate access to eligible and necessary services. Provides training and technical assistance to both internal and external stakeholders in order to enhance communication and coordination within the juvenile systems of care. Acts as the single point of contact for information specific to member's entering and exiting the children's services system.

Responsibilities include:

• Serving as the single point of contact to communicate and collaborate with children serving systems.
• Acting as the interagency liaison between the Children, Youth, and Families Department, the members, their families/caregivers, and other organizations.
• Ensuring appropriate transitions both in and out of the State Custody, including care coordination and completing documentation within the member's record.
• Partnering with clinical and quality teams to conduct outreach to child-serving higher levels of care.
• Interfacing with children's advocates to support development of a care plan that takes the child's needs into consideration.
• Communicating with Medicaid as needed.
• Sharing information that assists the clinical team in developing treatment plans that incorporate children's and their caregivers needs.
• Coordinating with providers to ensure appropriate access to care and follow-up.
• Developing and initiating collaborative interagency protocols and MOU agreements within the both community-based and congregate care providers.
• Providing coverage in the absence of the Care Coordinator.

Other job requirements include:

• Bachelor's degree in Social Work, Nursing, Psychology, or related field required.
• 5+ years of experience working within the children's behavioral health system or with child serving agencies.
• Strong verbal and written communication skills.
• Demonstrated problem solving skills.
• Organized with strong attention to detail.

Salary range: $58,440 - $93,500. This position may be eligible for short-term incentives as well as a comprehensive benefits package.
