Join our team as a Social Services Coordinator and make an impact by coordinating outside psychiatric and psychosocial services to assist with behavior modification plans, ensuring comprehensive care for residents. As a social services coordinator you will assess resident needs through social service interviews, provide counseling, advocacy, discharge planning, and access to community resources. The social services coordinator will encourage resident participation in individual and group programs, both inside and outside of the facility. You will facilitate family involvement in care plans and assist residents with legal documents, ensuring seamless transitions and ongoing family support. Additionally, you will meticulously document social service plans and progress notes in resident charts, reflecting goals and interventions effectively.
Schedule: This position is full time Monday- Friday from 8am-5pm.
