Essential Functions:

ThriveKids Student Wellness is a community-based program at Children's Hospital New Orleans with a mission to improve healthcare and academic outcomes for students by partnering with schools to expand access to healthcare services; services include staffing schools with healthcare professionals and providing/facilitating direct patient care services for schools. For this position, individual will work as a member of the ThriveKids Care Coordination Team providing care coordination services to youth enrolled in Thrivekids. Individual will have access to Children's Hospital New Orleans specialty and behavioral health services to supplement and resource student health needs. Care coordinator will also assist in developing a comprehensive guide to external/community behavioral health resources for families. Individual will be a full-time direct employee of Children's Hospital New Orleans and will have the option of a 10-month or 12-month working schedule (compensation adjusted respectively). As a support resource to school partners, individual will serve as a communication liaison between medical providers, families, and school personnel. Individual will facilitate re-entry meetings for Thrivekids enrollees that are re-entering school following an extended crisis, extended hospital admission, or other high acuity absence from school to ensure school partners are aware of all follow up care. Insight gained in the field will contribute to ThriveKids behavioral health programmatic development. Care coordinator has the opportunity for career development at Children's Hospital New Orleans as well as the option to provide student/patient counseling as needed. May require travel to individual schools as needed.

Education Qualifications

Master's degree from an accredited school in Social Work. Licensed in the State of Louisiana. Participation in appropriate continuing education, annual education requirements, and to accept personal responsibility for other educational activities to enhance job-related skills and abilities.

Education Qualifications

Experience or training in providing services to children and families. Must have excellent skills in communication, the ability to work both independently and in a team environment, and to prioritize. Must have clinical knowledge in order to assess the needs of youth enrolled in Thrivekids.

Licenses And Certifications

LMSW/LCSW, maintains current driver's license and care insurance.

We seek detail-oriented, independent self-starters who demonstrate an unwavering commitment to improving student health and raising student achievement. Must have working knowledge of federal and state laws, policies and regulations relevant to schools. Must have a record of high performance in challenging situations, requiring problem solving skills and good judgement. Must have good time management, communication (oral and written), conflict management, organizational and planning skills. Ability to maintain key community partnerships through excellent people skills, demonstration of accountability, and self-awareness. Ability to work collaboratively with other professionals to obtain positive solutions. Must possess strong critical thinking skills and willingness to contribute to program development.
