Care Support Specialist I

This role enables associates to work virtually full-time, with the exception of required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Please note that per our policy on hybrid/virtual/work, candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment, unless an accommodation is granted as required by law. An 8 hour shift between the hours of 8 am - 6 pm CST.

Carelon is the nation's largest community-based palliative care provider, currently operating in 25 states and 67 cities. Carelon specializes in providing an extra layer of support to patients facing a serious illness through an ambitious palliative care program focused on caring for patients in their homes and outpatient clinics. Carelon's clinicians are experts in providing patients with relief from the symptoms, pain, and stress of a serious illness; helping patients and caregivers navigate the healthcare system; guiding patients and caregivers through difficult and complex treatment choices; and providing emotional and spiritual support to patients and their families.

The Care Support Specialist is responsible for proactive and/or retroactive patient scheduling and serves as a catalyst for helping Carelon achieve its mission "to develop the nation's leading network of specialized physician practices that provides the highest quality medical, emotional, and spiritual support to patients and families facing a serious illness". Our Care Support Specialists demonstrate compassion for individual needs, attention to detail, and professional collaboration with other associates; all of which are critical for the successful execution of this role.

How will you make an impact:

  • Scheduling patients so that home-based provider availability is maximized and to ensure that patients are seen at the right time.
  • Provide support to providers by answering emails and inbound call transfers.
  • Support the transition of care process.
  • Assist providers and patients with HEDIS gap closures and HRA compliance.
  • Associates in this role are expected to have the ability to multi-task, including handling calls, texts, facsimiles, and electronic queues, while simultaneously taking notes and speaking to customers.
  • Additional expectations to include but not limited to: Proficient in maintaining focus during extended periods of sitting and handling multiple tasks in a fast-paced, high-pressure environment; strong verbal and written communication skills, both with virtual and in-person interactions; attentive to details, critical thinker, and a problem-solver; demonstrates empathy and persistence to resolve caller issues completely; comfort and proficiency with digital tools and platforms to enhance productivity and minimize manual efforts.
  • Associates in this role will have a structured work schedule with occasional overtime or flexibility based on business needs, including the ability to work from the office as necessary.
  • Performs other duties as assigned.

Minimum Requirements:

  • Requires a High School diploma or GED and up to 1 year of experience in an automated customer service environment; or any combination of education and experience which would provide an equivalent background.

Preferred Skills, Capabilities and Qualifications:

  • Associates or Bachelors degree preferred.
  • Working knowledge of the healthcare industry is strongly preferred.
  • Strong customer service skills.
  • Empathy for patient care.
  • Experience with scheduling, especially using Epic.
  • Background in home-health scheduling or route mapping.
  • Familiarity with Microsoft Word.
  • Tech savvy.
  • Call center experience, primarily outbound with some inbound calls.

For candidates working in person or virtually in the below location(s), the salary* range for this specific position is $16.55 to $28.75.
