Family Preservation Services Position

Family Preservation Services (FPS) provides intensive in-home services and case management services to families who have one or more children at risk of out-of-home placement or who will be at risk of out-of-home placement at birth.

Position Overview:

• Provides for the safety, stability and well-being of children and families during the referral period
• Delivers and coordinates culturally competent therapeutic and case management services to children and families
• Facilitates family meetings within required timeframes
• Engages and works in partnership with families to complete assessments and develop case plan goals and objectives
• Responds promptly to family crisis situations, documents critical incidents and is available 24/7 to clients
• Completes an initial and on-going family assessment related to safety, stability and well-being including mental health, substance abuse, education, emotional, social and physical well-being, parenting and parent-child relationships
• Assesses each child\'s safety with every family contact and reports all suspected child abuse/neglect
• Monitors family progress and service provision through the use of assessment tools, and updates family case plans based on those assessments
• Appears and testifies in court as required
• Provides 24 hour, 7 days a week support to clients and remains available during crisis situations
