Facilitator of Family Outreach Services/Juvenile Court School Liaison

Receives Guidance From: Building administration, district administration
Purpose of the Position: To provide long-term coordination of support services to children, youth, and their families who live in conditions which place the children and youth at risk of not succeeding in school.

Skills, Knowledge, and Abilities:
- Expertise and experience in working with a variety of persons and agencies to achieve common goals.
- Expertise and experience with school/community and interagency collaboration.
- Ability to identify risk and protective factor in working with families and students.
- Extensive knowledge of local resources.
- Understanding and knowledge of comprehensive family support services and professional referral services.
- Background that is sensitive to the needs of diverse populations with limited resources.
- Ability to work effectively as a member of a multi-disciplinary team, working cooperatively with both school and agency staff.
- Excellent written and oral communication skills.
- Excellent organizational and priority setting skills.
- Ability to organize and think clearly while communicating, organizing and evaluating.
- Ability to clearly articulate goals and expectations to adults and students.
- Knowledge of district organizational structure and function, as well as laws, rules, and regulations pertaining thereto.
- Ability to organize and prioritize workload with minimal supervision.
Education:
Certification/Licensure:
- Certification/Licensure in Social Work or Human Services preferred.
- Valid Iowa driver's license.
Experience:
- Experience and expertise in human services, counseling services, and/or case management.
- Experience with interagency collaboration.
- Experience with the juvenile court system preferred.
Equipment:
- Office equipment such as computers, two-way radios, multi-line phone system, cell phone, copier, and fax machines.
Essential Duties and Responsibilities
The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required and assigned:
- Engage in direct service support of students and families to increase academic success.
- Provide students and families with information and referrals to local and nearby resources.
- Attend student study team and other similar meetings for the purpose of communication and service coordination.
- Develop, implement and continuously monitor case plan with at-risk student/family.
- Assist students and their families in overcoming barriers to utilizing services.
- Provide emergency transportation for families.
- Develop, coordinate, and provide workshops and support groups for students and families in conjunction with district staff and agency representatives.
- Communicate information to school administrators and staff.
- Prepare and maintain paperwork for the district, including annual reports.
- Maintain confidential records for all activities and confidentiality with volunteer/community members.
- Monitor attendance with school team; implement intervention for chronic offenders.
- Collaborate with school personnel, families and outside agencies to plan for and provide social, emotional and behavior interventions and supports.
- Create and maintain a cooperative, nurturing, consensus building service environment.
- Participate in staff development opportunities within and outside the district.
- Attend conferences, seminars, and conventions appropriate to district needs and for professional growth.
- Keep current of family support services and educational development research.
- Maintain a reliable attendance record.
- Maintain confidentiality.

None
Physical Requirements
Ability to work while standing, moving and/or walking. Drug, tobacco, and alcohol-free while working. Must be able to see and hear within normal limits with or without corrective lenses or hearing aids. Ability to operate a motor vehicle used to travel between sites.
Working Conditions
Majority of work is performed in a school or community setting. Occasional exposure to bodily fluids, fumes, noise, and hazardous materials and emergency situations. Extensive district travel required. Some evening and weekend hours may be required.
