Job Summary: This position is located in any direct service division of Integral Care. The Service Coordinator performs moderately complex case management/service coordination work assisting clients in accessing/linkage to medical, social, educational, financial, and other appropriate services/supports that may help individuals achieve a quality of life and community participation acceptable to the individual/legally authorized representative (LAR). This position works under general supervision with moderate latitude for the use of initiative and independent judgment.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education: High School Diploma required. Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field preferred.

License: Current and valid driver's license and a clean driving record required.

Hours: Full-Time.

Workdays: Monday - Friday.

Workhours: 8:00AM - 5:00PM.

Salary: $24.18 - $25.39 an hour, based on education.
